The Medicare prescription drug program, known as Medicare Part D, provides a voluntary outpatient prescription drug benefit for Medicare beneficiaries. Beneficiaries may choose Part D plans from among multiple plans offered by private companies (plan sponsors) that contract with the Centers for Medicare & Medicaid Services (CMS). Plans may differ in their premiums and cost-sharing arrangements, the drugs they cover, and the pharmacies they contract with to fill prescriptions. CMS developed the Medicare Plan Finder interactive website in 2005 as a tool to help beneficiaries compare Part D plans and identify plans that meet their needs. For Plan Finder to serve its intended purpose, beneficiaries and their advisers need to be able to obtain accurate drug cost information, understand plan options, and navigate the website effectively. This report examines (1) how CMS oversees the accuracy of drug pricing information in Plan Finder; and (2) how CMS assesses the usability of Plan Finder and any steps CMS has taken to improve it. Malnutrition in acute care is well documented. In literature published since the early 1970s numerous studies have documented the prevalence of malnutrition in hospitalized patients. Although the reported incidence of malnutrition in acute care patients varies from study to study, the numbers are consistently high, ranging between 40 and 60%, regardless of age, diagnosis, or medical service. The consequences of malnutrition range from loss of lean body mass and longer hospital stays, to increased risk of pressure ulcers, increased morbidity, and increased mortality. Evidence shows that nutrition assessment and nutrition intervention can help reduce the consequences of malnutrition. One of the tools used for assessing nutrition status and the risk of malnutrition is the calorie count, which can determine whether a patient is meeting his or her nutrition needs via oral food intake or whether the patient requires nutrition support. Calorie counts in acute care are often conducted by dietary staff who are not properly trained to accurately assess a patient''s intake. The purpose of this project was to develop a nutrition training program for dietary staff to educate them regarding the importance of and correct process for completing calorie counts, and provide the training to the staff. Related literature shows that the common method of estimating intake, the "overall" estimation method, significantly overestimated intake for the majority of residents and failed to identify residents with inadequate intake 65% of the time. However, when tray items were estimated individually using a Food and Fluid Estimation Diagram, trained observers were able to accurately estimate intake 85% of the time. The instrument of education used for the current study was based on the Food and Fluid Estimation Diagram using digital photography of menu items at various stages of consumption as a reference. Two in-service sessions were conducted with a total of 18 participants; each session was attended by 9 participants. A total of 14 participants consented to allow their data on the pre-test calorie count and the post-test calorie count to be used for this study, resulting in 10 usable data sets. The average pre-test score was 76.8% and the average post-test was 77.1% showing a negligible increase of .3%. However, when the data are separated into session sets, clear differences in pre-test and post-test scores are observed. The morning session had a pre-test average score of 75.8% and a post-test average score of 79.4%. Five of the seven morning group participants improved their average test scores. The afternoon session had a pre-test average score of 79.1% and a post-test score of 71.9%. All three of the afternoon group participants showed a decrease in their average scores. It is possible that the afternoon group results were influenced by a disruption to the in-service education. When considered as a whole, there was a negligible increase in the accuracy of calorie counts. However, when the results are considered as separate sessions, the morning group indicated a positive effect from the education. These results confirm the positive finding of other research conducted on the accuracy of food intake estimates, indicating that improved accuracy can be obtained with education and training.
