The COVID-19 pandemic has highlighted that access to timely health spending data is crucial for informed policy-making. This Health Working Paper summarises and compares the methodologies applied in around half of OECD countries to estimate public and private health spending for the most recent year (i.e., t-1) as well as the approaches taken by the OECD Secretariat to fill existing data gaps for the remaining OECD countries. For the first time, the paper also explores the feasibility of nowcasting health spending for the current year (i.e., t) and examines data sources that could be potentially useful in such an exercise. While this review should help OECD countries that do not yet have experience in estimating health spending for year t-1 to improve the timeliness in their data reporting, the paper also analyses the applicability of the methods in low- and middle-income countries.

Admiral Wilhelm Canaris was the head of the Abwehr?Hitler''s intelligence service?from 1935 to 1944. Initially a supporter of Hitler, Canaris came to vigorously oppose his policies and practices and worked secretly throughout the war to overthrow the regime. Near the end of the war, secret documents were discovered that implicated Canaris and hinted at the extent of the activities conducted by Canaris''s Abwehr against the Hitler regime, and in 1945 Canaris was executed as a national traitor. But Canaris left little in the way of personal documents, and to this day he remains a figure shrouded in mystery. IntroductionThe conventional procedure of sentinel lymph node (SLN) mapping in vulvar cancer includes peritumoral injection of technetium-99m radiocolloid (Tc-99m) the day before surgery and intraoperative injection of methylene blue. However, these techniques have some weaknesses. Near infrared (NIR) fluorescence imaging with indocyanine green (ICG) has recently gained popularity in SLN mapping in different types of cancer. The aim of this study is to evaluate the clinical value of ICG SLN mapping in patients with vulvar cancer.MethodsWe retrospectively analyzed all patients at our institution with vulvar cancer undergoing SLN mapping using NIR fluorenscence imaging with ICG by applying video telescope operating microscope (VITOM) system technology between April 2013 and May 2019.Results55 groins of 28 patients were analyzed. In 44 groins we used Tc-99m, in four methylene blue and in five both techniques additionally to ICG for SLN detection. ICG was injected peritumorally directly before surgery and no complications due to its administration occurred. 104 SLNs were identified and removed. SLN detection rates were 87.3%, 87.8%, 77.8% and 96.4% for ICG, Tc-99m, methylene blue and the combination of ICG plus Tc-99m, respectively. SLN detection rates of ICG were significantly higher in patients with positive lymph nodes and lymphatic space invasion compared to Tc-99m (p-value 0.035 and 0.005, respectively). We further observed a higher detection rate of ICG in obese patients (94.4% vs. 87.5%); however, this observation is non significant (p-value 0.69).DiscussionAccurate SLN mapping is a crucial part of vulvar cancer staging and enables avoiding unnecessary inguinofemoral lymphadenectomies. SLN mapping with ICG in vulvar cancer is feasible and safe with a detection rate comparable to Tc-99m, particularly in patients with positive lymph nodes, lymphatic space invasion or obesity. Furthermore, it is easier to perform than mapping with Tc-99m and has less side effects compared to methylene blue.

Recent studies (Cavanaugh & Rademacher, 2014; Clapham et al., 2014; 2015; Stuhl & Porter, 2015) indicate that surfing can be an effective therapy for children with Autism Spectrum Disorder (ASD). The type of aerobic fitness associated with regular surfing may also promote brain development and cognitive function (Khan & Hillman, 2014; Segalowitz, 2016). Brown et al. (2012) suggest the need for community programs that focus on recreational activities to promote friendship development for children with ASD. Considering the studies showing a demand for social supports (Siklos & Kerns, 2006) and the limited access to social and recreational activities for school-aged children with ASD (Brown et al., 2012), the researcher has attempted to create a surf therapy program designed to teach social skills to children who have ASD. The included lesson plan sequence incorporates social development activities into a training program geared to develop the paddling and balancing skills associated with surfing. Throughout the program, students will learn about water safety, surf culture, surf history, paddling and surfing techniques, and worldwide surf destinations. The main intent of this program is to promote the availability of surf-focused recreational therapy programs (RTPs) for children with ASD by discussing the indicated social skills benefits that such therapies may have. The attached program is intended to provide children who have ASD with an opportunity to practice important social skills while learning about the various dynamics of surfing. This paper studies the causal impact of court deferrals, a legal strategy to help defendants avoid a felony conviction record, on the future criminal and labor market outcomes of first-time felony drug offenders. To accomplish this, we exploit two natural experiments in Harris County, Texas, in which defendants appearing in court one day versus the next experienced abruptly different likelihoods of deferral. In 1994 deferral rates dropped by 34 percentage points the day following the implementation of a penal code reform; in 2007 deferral rates increased by 22 percentage points the day after the unexpected failure of a ballot initiative to expand the county jail. Using administrative data and local polynomial regression discontinuity methods, we find robust evidence consistent across both experiments that regimes with expanded use of court deferrals generated substantially lower rates of reoffending and unemployment over a five-year follow-up period. Additional analysis delves further into the timing, nature and incidence of these impacts. Together our results suggest that increasing the use of deferral programs may be an attractive and feasible option for a jurisdiction seeking to reduce the fiscal cost and community impact of its criminal justice system.

Intermittent renewable energy sources do not always provide power at times of greatest electricity demand or highest prices. To do so reliably, energy storage is likely required. However, no single energy storage technology is dominant when comparing cost intensities of the energy capacity and power capacity of storage. Past research on energy storage technologies has debated the value of storage technologies for different applications, and has compared the cost structures of different storage technologies without finding generalizable results across both locations and technologies. Here, a single performance metric, the benefit / cost ratio (X) of storage value added is analyzed across six locations globally to show that the relative value of storage technologies is largely location invariant. Electricity price dynamics, specifically the frequency and height of price spikes determine the value of storage, while the duration of price spikes determines the relative value of one technology versus another. We find that cost targets can be set for different technologies with ranging energy and power costs of storage.
