The goal of the present study was to perform a partial test of the cognitive components in Mendez and Cummings' (2003) three of nine modular component failure rule for the diagnosis of dementia syndromes. They argue that a diagnosis of dementia can be made when a modular failure occurs in any three of the following nine areas: language, memory, perception, praxis, calculations, conceptual or semantic knowledge, executive functions, personality or social behavior, and emotional awareness or expression. In this study we evaluated the cognitive variables of language, memory, perception, and conceptual or semantic knowledge. The study sample included 338 veterans with a heterogeneous mixture of neurologic and psychiatric disorders who received a neuropsychological evaluation. A frequency count of modular failures and diagnostic efficiency statistics were used to compare the behavioral neurologic model for the diagnosis of dementia to that of the psychiatric model for the diagnosis of dementia. In general the behavioral neurologic model had excellent specificity, with type I errors meeting statistical significance. However, the neurologic model's sensitivity was less robust and produced type II errors that did not meet statistical significance. Moreover, the psychiatric diagnostic model requires a modular failure to occur in memory before a diagnosis of dementia can be made. The behavioral neurologic model does not require a modular failure in memory to occur, instead it requires three modular failures to occur before a diagnosis of dementia can be made. The difference in how the two models operationally define dementia allowed the behavioral neurologic model to identify one individual who did not have a memory impairment, but was diagnosed with dementia because three modular failures occurred. However because the behavioral neurologic model requires three modular failures, 58 individuals in our study who were diagnosed with dementia using the psychiatric model were not eligible for consideration of a dementia diagnosis using the behavioral neurologic model. Overall, because of the high false negative frequency, the psychiatric model should remain the diagnostic standard for the evaluation of dementia, at least until a complete test of the behavioral neurologic model is conducted.
