Ben Nakadai has never embraced the adventure of business travel. His current trip to Japan is no exception. On his first night in the land of his ancestors, he has a restless sleep and dreams a cryptic dream that haunts him the next morning. In the hotel bar, he meets the mysterious Mykoto, who informs Ben that Nakadai is a storied name in Japan. Mykoto embarks on a tale of Ben''s samurai ancestors, but the story abruptly ends when Ben receives a phone call. While reflecting on Mykoto''s story, a violent thunderstorm erupts and it may well be a harbinger of violence to come.

While Wanda lies close to death, Spawn returns to the eighth level of Hell for a final showdown with a weakened Malebolgia. At the height of the conflict, Angela arrives to join in the battle. After mortally wounding Malebolgia and thinking him dead, Spawn and Angela prepare to escape; however, with his last breath, Malebolgia extracts Angela''s lance and impales her with it. In a rage, Spawn decapitates Malebolgia with Angela''s sword. Carrying Angela''s lifeless body, Spawn delivers her to a host of angels who offer him forgiveness and redemption. Spawn is led to a former church, now an archive of Hellspawn memorabilia. There, Cog tells Spawn of his own life as a Hellspawn, and how Spawn may break his curse. Meanwhile, detectives Burke and Twitch go looking for Spawn. After finding no sign of Spawn in the alleys, the two are confronted by a stranger. Also, Bobby is in despair, practically at rock bottom. After taking a bottle from a murdered drunk, he asks Heaven''s forgiveness.

Sam and Twitch ponder the results of their research on Lt. Colonel Al Simmons. Meanwhile, Spawn continues to battle the Heap until he ends up engulfed by the mass of trash just as Cog and Bootsy arrive to save him. When Bootsy kneels to pray for help, he finds the sigil of the Hellspawn, a token signifying that Spawn is indeed gone. Elsewhere, Spawn falls into a dark space where the former Eddie Beckett awaits for him and points him toward a light source where he is welcomed "home."

Twitch is able to break free from his captor, the Freak, when he is distracted by Spawn''s sudden appearance. Then, as Sam, Twitch & Spawn struggle to stop the slaughter in the alley, they feel their curious bond intensifying. Spawn tracks down the Freak, where he is drowned by the worms and insects. Meanwhile, Wanda again witnesses Cyan''s psychotic behavior when she loses her soother and necklace out the car window.

After Cyan is returned safely to her parents, Al has a long talk with Granny Blake and as usual, her words of wisdom calm him while creating inner confusion. When he gets back to the alley, the alley bums have rebuilt his throne which seems to hypnotize him as he settles into it. The Clown appears in Spawn''s trance and reveals startling facts about his past and future. Meanwhile, Wynn confrims Cy-Gor''s remains are safe at a testing facility and orders the disappearance of Terry.

Spawn freefalls through the levels of Hell where he he undergoes a physical and psychological baptism. Layers of Al Simmons'' life are peeled away and his weaknesses are exposed. Finally, Malebolgia rips Al''s heart out. Meanwhile, Terry''s doctors are still puzzled by his miraculous recovery while Cyan refuses to give up a filthy shoelace she found at the hospital. Elsewhere, Sam shows Twitch their new ''55 Chevy car.

At the police department, Burke threatens to expose Chief Banks with the file Spawn gave him, which links Banks to the child killer Kincaid. Banks faces Spawn, but loses his nerve and reveals that he works for Wynn. Enraged, Spawn confronts Wynn but Terry bursts into the office and demands Wynn''s release. Spawn believes Terry has befriended Wynn.

While a strange assailant stalks the city, ripping out human hearts, another otherwordly being arrives. As his mind reels, our tortured hero remembers that he struck a deal with the devil in order to return to his beloved wife - five years after his death.
