Near death, Nyx makes final preparations for her trip into the depths of Hell to save her friend, Thea. With Al Simmons in the hospital after his last encounter with the Clown and the thousand of his minions, his condition goes from bad to worse once Nyx takes away his Hell-born essence. With the powers of a Hellspawn at her whim, Nyx strikes a deal with N'Zzezheaal, a lessor lord of Hell, in hopes of gaining free passage into the 8th level of Hell. Once there, Nyx is determined save her friend. But who or what is going to save Nyx?

Nyx persuades Al that he should try to right the many sinister deeds he has been responsible for, and starts by going back to the alleys to save Major Forsburg. Triumphantly, Jason Wynn and his new "co-worker" return to the CIA offices. Jason goes right to work, and asks for all the files on Al Simmons. At police headquarters, Twitch discovers that the man who had no shadow was also the man who's grave had been robbed, Al Simmons.

Arising out of young Eddie''s search for revenge on the Hellspawn, the Redeemer finds Spawn in the desert and intends to finally settle the score. As the battle between the two is taking place, the mysterious stranger visits Eddie''s bedside in the hospital and reveals the truth to an unconscious Eddie that has always been impossible for him to admit: His father was an abusive, drunken brute. While the truth could kill Eddie, it has the Redeemer examining his own motives. After receiving a final crushing blow from the Redeemer, Spawn lays on the desert floor while two patiently waiting scorpions do what they have been sent to do: drag the Hellspawn down into the bowels of Hell.

While Wanda lies close to death, Spawn returns to the eighth level of Hell for a final showdown with a weakened Malebolgia. At the height of the conflict, Angela arrives to join in the battle. After mortally wounding Malebolgia and thinking him dead, Spawn and Angela prepare to escape; however, with his last breath, Malebolgia extracts Angela''s lance and impales her with it. In a rage, Spawn decapitates Malebolgia with Angela''s sword. Carrying Angela''s lifeless body, Spawn delivers her to a host of angels who offer him forgiveness and redemption. He refuses this angelic pardon, but the angel who offers it can yet do one favor for Spawn: She appears in Wanda'' hospital room and breathes new life into Wanda. Meanwhile, Spawn finally comes to terms with his place in the world.

Ethan Crone, accused by his editor of being obsessed with Spawn, interviews people from all walks of life around the city who may have witnessed Spawn and his mysterious ways. When he actually tries to spend the night in the Rat City alleys, Cog tells him that there is no Spawn and there never was: he was simply a ghost story told around the bonfires in the alleys.

Spawn is led to a former church, now an archive of Hellspawn memorabilia. There, Cog tells Spawn of his own life as a Hellspawn, and how Spawn may break his curse. Meanwhile, detectives Burke and Twitch go looking for Spawn. After finding no sign of Spawn in the alleys, the two are confronted by a stranger. Also, Bobby is in despair, practically at rock bottom. After taking a bottle from a murdered drunk, he asks Heaven''s forgiveness.

Bootsy steps between Spawn and Wolfram, the vampire, to keep them from destroying each other. When Wolfram mentions a nest of vampires in the area, Spawn goes to investigate just as the police arrive and take Bootsy in for questioning. At the police station, Bootsy passes a cryptic message to Sam and Twitch. Elsewhere in an abandoned warehouse, a child is being tortured. Spawn breaks in, kills the abusers and releases the child who reveals an emblem blazoned across his belly.

When Cog promises to answer Sam and Twitch''s questions, they follow him into the deepest parts of Rat City. Twitch takes notes as Cog tells the tale of Spawn''s confused existence as it relates to them. They view Spawn''s throne and hear of his life as it was on earth before he returned as a Hellspawn. At the tour''s end, Cog tells the detectives that Spawn has chosen them to help recapture his lost humanity.

A shaken Jason Wynn tells his security forces about a "terrorist threat" on his life and they reinforce the area. Meanwhile, Spawn's face has healed itself. Although startled and confused by this new turn of events, Al forms plans to further intimidate Wynn to ensure Wanda's safety and to win her back. Later, Cog counsels Spawn to get him to reconsider his decisions. Elsewhere in the city, Sam & Twitch are pondering how some of their previous cases seem to be tied together when they receive a hot tip and head to the alley.

After Cyan is returned safely to her parents, Al has a long talk with Granny Blake and as usual, her words of wisdom calm him while creating inner confusion. When he gets back to the alley, the alley bums have rebuilt his throne which seems to hypnotize him as he settles into it. The Clown appears in Spawn''s trance and reveals startling facts about his past and future. Meanwhile, Wynn confrims Cy-Gor''s remains are safe at a testing facility and orders the disappearance of Terry.

Spawn freefalls through the levels of Hell where he he undergoes a physical and psychological baptism. Layers of Al Simmons' life are peeled away and his weaknesses are exposed. Finally, Malebolgia rips Al's heart out. Meanwhile, Terry's doctors are still puzzled by his miraculous recovery while Cyan refuses to give up a filthy shoelace she found at the hospital. Elsewhere, Sam shows Twitch their new '55 Chevy car.

When Angela helped Spawn escape Heaven, she lost her position as priority A-1 angel. Tiffany, a firecracker with a taste for blood, is determined to get the job; and her path to success leads straight through Spawn. Coming upon him in his weakened state, Tiffany confronts Spawn and a fierce battle ensues with Tiffany about to decapitate Spawn. Meanwhile, Sam and Twitch wonder why Banks took the fall when others were implicated in their investigation.

As Cy-Gor rampages, the Curse prepares to get to the root of Spawn''s evil by studying his body. Left alone, Spawn severs his own hand and uses it to crack the tank holding his uniform. The uniform escapes and goes berserk as it re-integrates its host while attacking the Curse, leading him to blow up the entire complex.
